Implement #29: optional VISL CG3 stream output from suggest Add a `format` option to the suggest command (default "json", unchanged). With `format = "cg"` the step emits a VISL CG3 stream instead of the error JSON: the input cohorts, readings and blanks are round-tripped verbatim via cg3-rs, and each cohort's generated suggestions are appended as a `→ form1, form2` line. Suggestions are produced with the same proc_reading path as the JSON output, so the dynamic-compound handling (#31) applies — a compound the generator can't build appends nothing rather than a bogus partial. Select per run with `-c 'suggestions={"format":"cg"}'`.
